Important Information Before Requesting an Appointment
Some new patients are required to provide additional medical records before an appointment can be scheduled.
This includes patients who:
- are transferring psychiatric care from another provider;
- have had a psychiatric hospitalization within the past 12 months; or
- are currently taking or requesting evaluation for stimulant medications, benzodiazepines, or other controlled substances.
