Before Scheduling Your First Appointment
At Elysium Mental Health, certain new patients must meet additional requirements before an appointment can be scheduled.
These requirements apply to all new patients meeting the criteria below and will not be waived.
Transfer of Psychiatric Care
Patients transferring care from another psychiatrist, psychiatric nurse practitioner, or other psychiatric provider must provide previous psychiatric records before an appointment can be scheduled.
Appointments will not be scheduled until the required records have been received.
Recent Psychiatric Hospitalization
Patients who have been psychiatrically hospitalized within the previous 12 months must:
- Provide the hospital discharge summary and relevant hospital records before scheduling.
- Complete their initial psychiatric evaluation in person.
Appointments will not be scheduled until the required records have been received.
Controlled Medications
Patients currently taking or requesting evaluation for stimulant medications, benzodiazepines, or other controlled substances must:
- Provide documentation supporting the diagnosis and previous treatment before scheduling.
- Complete their initial psychiatric evaluation in person.
Providing previous records does not guarantee continuation of any medication.
All prescribing decisions are made only after an independent psychiatric evaluation and the clinical judgment of the treating psychiatrist.
Elysium Mental Health does not provide automatic refills or temporary (“bridge”) prescriptions for controlled substances before the required evaluation has been completed.
